Military Treatment Facility

Medical care foractive duty personnel, military retirees, eligible veterans and their enrolled family members are provided by the61st Medical Squadronat the Los Angeles AFB Clinic, located in Bldg. 210 near the Main Gate off Douglas Street. A satellite clinic is also located in the Fort MacArthur military housing area.

Emergency medical care is not available at either clinic. For life threatening injuries or illnesses, call 911 or proceed to the nearest Emergency Room or Urgent Care Center listed here.

*The clinic is also closed the first Thursday of every month in the morning for currency training. It reopens at 1 p.m.

Nurse Advice Line

Sometimes it is difficult to know if and when to seek medical help for acute health problems, so having professional help at a moment's notice is invaluable. The Military Health System's new Nurse Advice Line for TRICARE beneficiaries does just that, toll-free, 24 hours a day, seven days a week by dialing 1-800-TRICARE (874-2273); Option 1.

The NAL is a team of registered nurses who are available to answer a variety of urgent healthcare questions. They can help you decide whether self-care is the best option, or if it is better to see a healthcare provider. There will always be a live-person on the line to address beneficiary concerns.

The NAL offers a variety of solutions for all TRICARE beneficiaries. When necessary, the NAL can make same-day appointments with the beneficiary's PCM for beneficiaries enrolled to the MTF. If a same-day appointment is not available, the NAL can re-direct the beneficiary to the closest urgent care center. All other TRICARE beneficiaries who are not enrolled to a MTF will receive professional health advice about their urgent health concern and when to seek urgent care.

When calling the NAL, a customer service representative will verify the beneficiary's eligibility through the Defense Enrollment and Eligibility Reporting System. Those with an acute health care concern or question will be connected with a registered nurse that will ask the beneficiary a series of very standard questions to determine the next steps and allow the NAL nurse to provide the best advice possible. Beneficiaries can still call their PCM or clinic, but the NAL is another option for accessing the care in a timely fashion.

App Rescheduling

Sometimes there are conflicts that may prevent you from attending your scheduled appointment. If this happens, the clinic respectfully asks that you call to cancel or reschedule your appointments 24 hours in advance if you cannot make it so we can offer your appointment to another patient. If you need to cancel or reschedule your appointment, call (310) 653-CURE during regular business hours. If booked via TRICARE Online, medical appointments can also be cancelled online.

A patient is considered late if they arrive more than 10 minutes after their scheduled appointment time. Don't be That Guy (or Gal)!

Patients who arrive late will either be categorized as a "No-Show", offered the opportunity to reschedule their appointment, or given the option to be seen after other scheduled patients at the discretion of the provider.

How does the Exceptional Family Member Program, Overseas Clearance and PCS work?

OCONUS PCS
Family Member Relocation Clearance process is required for all family members traveling.

AF Form 1466
· Pages 1-3: Completed and signed by Active Duty Member
· Pages 4-5: Leave Blank. This is completed at FMRC Appointment.

AF Form 1466D (Dental): Completed for all dependents ages 2 and above by a military or civilian dentist.

DD Form 2792 (Physician Form): Completed by physician (military or civilian) for all dependents. If care is provided by a specialist, please have the specialist complete form.

DD Form 2792-1 (School Form): Completed for all dependent children, by their school. If home- schooled, home school teacher completes. If not school aged, a parent can complete. If an Individual Education Plan (IEP) or Individual Family Service Plan (IFSP) exists, it must be included.
· If the family member is not seen at a Military Treatment Facility, please have the Civilian physician(s) provide a copy of all medical records for the past 18 months.

CONUS PCS
For family members enrolled in the Exceptional Family Member Program(EFMP) or meeting criteria for EFMP:

UnitedHealthcare Military & Veterans is the managed care support contactor for the TRICARE West Region. Under the new contract, beneficiaries will be required to file claims to new addresses, have new case managers if they had been assigned one by TriWest, and may need to find a new doctor, since United Health's civilian network differs from the former contractor, TriWest Healthcare Alliance.
