Los Angeles AFB Aerospace Medicine

All flying and special operational duty personnel must report to the Flight Medicine Clinic during their base in-processing and upon completing a clinical visit anywhere outside of Flight Medicine. Records must be hand-carried back to Flight Medicine immediately upon completing a visit to any other physician, caregiver, psychiatrist, psychologist, nurse practitioner, etc. This includes referrals outside the 61st Medical Squadron. Should you require medical care during non-duty hours, don't delay care if you feel it's necessary. However, you must notify the Flight Medicine Clinic the first duty day following your visit to another health care office or clinic. This includes Emergency Room and Urgent Care clinic visits.

The Flight Medicine clinic is open from 7 a.m. - 3:30 p.m., Monday-Friday, excluding holidays and training days on the 1st Thursday of the month. All patients are seen by appointment only and should schedule appointments (routine and sick call) by calling (310) 653-2873, DSN: 633-2873 or the Flight Medicine front desk at (310) 653-6850 DSN: 633-6850. Public Health Office

For Deployment Medicine support, please walk in on Mondays and Tuesdays 7 a.m. to 4 p.m. Otherwise call for an appointment.

For all other Public Health services please walk in Monday through Friday, 7 a.m., no appointment required.

The office is closed on weekends and the first Thursday of every month.

Some of the services offered by Public Health include: Deployment and Travel Medicine, Occupational Health Surveillance and Education, Communicable Disease Surveillance and Education, Food and Sanitation Surveillance (including base fundraisers), Medical Clearances and Security Clearance processing, and Duty Limitations/Medical Evaluation boards.

PHA Clinic

A Preventive Health Assessment (PHA) is an annual requirement for all active duty personnel. In order to avoid delays in completing your PHA you MUST complete the Air Force Web-based Health Assessment within 30 days of your appointment. To complete the assessment, click on: https://www.defenserack.com/afwebha/login.asp , enter your social security number and date of birth to log in. The AF WEB HA website can be accessed from any computer with an internet connection. You may stop and start your questionnaire as many times as you need. You can log back in using your SSN and DOB. You will start from where you left off and not have to repeat any questions.

If required, you must also fast for 10-12 hours prior to reporting to the Lab to get your blood drawn. If you wear corrective lenses, please make sure to bring them to your appointment.

Failure to complete your annual PHA will negatively affect your individual medical readiness and ability to deploy.
